Посттравматическое стрессовое расстройство (ПТСР) — тяжелое психическое состояние, возникающее после психотравмирующих событий и приводящее к стойким нарушениям эмоциональной, когнитивной и поведенческой сфер. Актуальность его диагностики возрастает в условиях социально-экономических кризисов, военных конфликтов и роста числа пострадавших от насилия. Несвоевременное выявление ПТСР влечет серьезные последствия: соматические заболевания, снижение трудоспособности, социальную дезадаптацию и суицидальные риски.</p></sec><sec><title>Цель</title><p>Цель: анализ современных и перспективных методов диагностики посттравматического стрессового расстройства.</p></sec><sec><title>Результаты</title><p>Результаты. В основной части обзора рассматриваются современные методы диагностики ПТСР: 1. Традиционные клинические опросники и интервью, обладающие высокой валидностью, но ограниченные субъективностью и временными затратами; 2. Биомаркеры ПТСР: кортизол, ГАМК, провоспалительные цитокины, а также нейровизуализационные (фМРТ, ЭЭГ) и электрофизиологические (кожная проводимость, вариабельность сердечного ритма); 3. Выявление смещения внимания к угрожающим стимулам через анализ движений глаз и диаметра зрачка при помощи Айтрекинга; 4. Применение цифровых диагностических платформ (PTSD Coach, Coviu) для дистанционного скрининга и мониторинга симптомов ПТСР; 5. Модели машинного обучения, анализирующие текстовые, ЭЭГ- и генетические данные для повышения точности диагностики.</p></sec><sec><title>Выводы</title><p>Выводы. Перспективы развития диагностики ПТСР связаны с интеграцией мультимодальных подходов, включая биосенсоры для непрерывного мониторинга кортизола, портативные нейровизуализационные системы (инфракрасная спектроскопия) и персонализированные алгоритмы на основе ИИ. А также с решением методологических проблем — стандартизации методов, интерпретируемости моделей и этических аспектов.</p><p>Совершенствование диагностики ПТСР требует комбинации традиционных и инновационных методов, что позволит улучшить раннее выявление, терапию и качество жизни пациентов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Post-traumatic stress disorder (PTSD) is a severe mental condition that occurs after traumatic events and leads to persistent disturbances in emotional, cognitive, and behavioral functioning. The importance of its diagnosis is increasing in the context of socioeconomic crises, military conflicts, and the increasing number of victims of violence. Late detection of PTSD has serious consequences: somatic diseases, decreased ability to work, social maladjustment, and suicidal risk. Objective: To analyze current and emerging diagnostic methods for post-traumatic stress disorder.</p></sec><sec><title>Results</title><p>Results. The main part of the review examines current diagnostic methods for PTSD: 1. Traditional clinical questionnaires and interviews, which have high validity but are limited by subjectivity and time consumption; 2. PTSD biomarkers: cortisol, GABA, proinflammatory cytokines, as well as neuroimaging (fMRI, EEG) and electrophysiological (skin conductance, heart rate variability) measurements; 3. Detection of attentional bias toward threatening stimuli through eye movement and pupil diameter analysis using eye tracking; 4. Use of digital diagnostic platforms (PTSD Coach, Coviu) for remote screening and monitoring of PTSD symptoms; 5. Machine learning models analyzing text, EEG, and genetic data to improve diagnostic accuracy.</p></sec><sec><title>Conclusions</title><p>Conclusions. Prospects for the development of PTSD diagnostics are linked to the integration of multimodal approaches, including biosensors for continuous cortisol monitoring, portable neuroimaging systems (infrared spectroscopy), and personalized AI-based algorithms.
