Во-первых, наличие коморбидных состояний существенно усложняет диагностический процесс. Значительное перекрытие симптомов различных психических расстройств способно обусловливать дифференциально- диагностическим сложностям и даже к искусственной коморбидности. Во-вторых, коморбидные психические расстройства обычно ассоциируются с более тяжелым течением, худшим ответом на лечение, повышенным риском рецидивов и менее благоприятным прогнозом.</p><sec><title>Цель</title><p>Цель: исследование проблемы коморбидности посттравматического стрессового расстройства (ПТСР) с другими психическими расстройствами, включая аффективные, тревожные, психотические и аддиктивные расстройства.</p></sec><sec><title>Результаты</title><p>Результаты. В статье рассматривается проблема коморбидности посттравматического стрессового расстройства (ПТСР) с другими психическими расстройствами, включая аффективные, тревожные, психотические и аддиктивные расстройства. Подчеркивается высокая распространенность сочетанных форм патологии, их негативное влияние на клиническое течение, эффективность терапии и социальную адаптацию пациентов. Описаны эпидемиологические данные, нейробиологические механизмы (снижение объема гиппокампа, дисфункция миндалевидного тела, нарушения регуляции HPA-оси), а также психологические факторы коморбидности, такие как эмоциональная дисрегуляция и избегающее поведение. Особое внимание уделяется сложностям диагностики из-за симптоматического перекрытия и необходимости использования структурированных интервью. Обсуждаются терапевтические стратегии, включая фармакотерапию и психотерапию, и подчеркивается важность интегративного подхода. Делается вывод о необходимости трансдиагностического моделирования и интеграции психиатрической помощи в систему здравоохранения.</p></sec><sec><title>Выводы</title><p>Выводы. Коморбидность ПТСР с другими психическими расстройствами представляет собой не только частое клиническое явление, но и сложную проблему, требующую комплексного подхода на индивидуальном, системном и социальном уровнях. Улучшение понимания природы этих сочетаний, оптимизация диагностики и лечение, а также разработка профилактических стратегий являются ключевыми задачами современной психиатрии.</p></sec></abstract><trans-abstract xml:lang="en"><p>Relevance of comorbidity research in psychiatry is determined by several key factors. First, the presence of comorbid conditions significantly complicates the diagnostic process. Significant overlap in symptoms across various mental disorders can lead to differential diagnostic difficulties and even artificial comorbidity. Second, comorbid mental disorders are typically associated with a more severe course, poorer treatment response, increased risk of relapse, and a less favorable prognosis.</p><sec><title>Рurpose</title><p>Рurpose. To study the comorbidity of post-traumatic stress disorder (PTSD) with other mental disorders, including affective, anxiety, psychotic, and addictive disorders.</p></sec><sec><title>Results</title><p>Results. This article examines the comorbidity of post-traumatic stress disorder (PTSD) with other mental disorders, including affective, anxiety, psychotic, and addictive disorders. The high prevalence of comorbid conditions, their negative impact on the clinical course, treatment effectiveness, and social adaptation of patients, is highlighted. Epidemiological data, neurobiological mechanisms (reduced hippocampal volume, amygdala dysfunction, HPA axis dysfunction), and psychological factors of comorbidity, such as emotional dysregulation and avoidant behavior, are described. Particular attention is paid to the diagnostic challenges due to symptomatic overlap and the need for structured interviews. Therapeutic strategies, including pharmacotherapy and psychotherapy, are discussed, emphasizing the importance of an integrative approach. A conclusion is drawn regarding the need for transdiagnostic modeling and the integration of psychiatric care into the healthcare system.</p></sec><sec><title>Conclusions</title><p>Conclusions. The comorbidity of PTSD with other mental disorders is not only a common clinical phenomenon but also a complex problem requiring a comprehensive approach at the individual, systemic, and societal levels.
